How do I request my medical records from Advocare?

Request Online:

The easiest and most convenient way to access your medical records is to request them online through Patient Request. CLICK HERE to submit a request to access your records anytime, anywhere. For a video tutorial, CLICK HERE. Please have your driver’s license or other government ID ready.
 
Request in Person:

To request your medical records in person, please complete the “AUTHORIZATION FOR USE/DISCLOSURE OF PROTECTED HEALTH INFORMATION” form, and return the form to your Care Center so that they can process your request.
