How do I request my medical records from Advocare?



If you are a patient of ours and would like to request your medical or billing records, please click the buttons below to complete your request. You will be required to provide a valid email address and a government-issued ID.


For status updates of your request, please consent to receive status notifications about the progress of your request via email and text (SMS).


Only the patient, a parent/legal guardian, or the patient’s legal healthcare representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for healthcare/ Conservatorship.

Request Online:

The easiest and most convenient way to access your medical records is to request them online through ShareCare. Click below to submit a request to access your records anytime, anywhere. Please have your driver’s license or other government ID ready and a valid email address.

Request In Person:

To request your medical records in person, please complete the “Authorization For Use/Disclosure of Protected Health Information” form, and return the form to your Care Center so that they can process your request.
